Paperwork is a necessary and unavoidable part of any surgical procedure or
treatment. In order to reduce the amount of paperwork day of surgery, please
complete both forms at home and bring them with you to your appointment.
Instructions:
1. Download both (2) pages to your computer by clicking on the link below.
This may take up to a minute or two if you use a slow dial-up connection.
2. Please Fill IN THE FIELDS on this form while still on your computer
screen, PRINT the completed form AND BRING TO YOUR SCHEDULED APPOINTMENT.
NOTE: Due to software limitations, the information you’ve filled out in the
fields on the forms will NOT be retained should you decide to “File/Send”
via e-mail the completed PDF file before printing it. So, please do NOT
e-mail completed forms to us.
3. Alternatively, you can fill out the forms by hand after first printing
the blank form.
